Requests should include the following:

1.  A stamped, addressed (my name and school's address for the return) envelope for each            recommendation

2.  Secondary recommendation forms with your information already filled out

PLEASE NOTE:

As a teacher of upper classmen, I always receive a lot of requests for recommendation letters.  They take a great deal of time to complete, so you should know that requests are accepted on a first come, first served basis.  I am also accepting only 40 requests, so get them to me early!
